Case Management
For members with complex health care needs
Lovelace Health Plan provides Case Management services to members with multiple or
complex health care needs. Case Managers coordinate care between physical health
and behavioral health, as well as assist members with community services, social services
or medical equipment needs in the home.
Case Management may be of assistance if you have one of the following needs/conditions:
- Cancer
- Neuromuscular diseases such as MS
- Multiple medical conditions
- Conditions requiring complex home equipment or medications
- Paraplegia and quadriplegia
- Organ transplant
- Conditions requiring coordination of out-of-network referrals and treatment services not available in-network
- Conditions requiring community resources and social services
- Those with complicated hospital discharges that could benefit from follow-up once they are home
- Members in remote areas who are unable to identify resources or providers for their medical care
- Infants who had complicated deliveries or birth defects
- Expectant mothers in need of support/coordination of services
- Members who might need additional support in order to prevent a hospital admission
- Hospice
- Assistance with long term placement
Who Can Make Referrals to Case Management?
Referrals for Case Management services come from multiple sources. However, Case Management will take place only with the consent of the member or the member’s power of attorney. Common sources of referral are:
- Members may self refer
- Member’s family
- Physicians
- Hospital Case Managers
- Community Case Managers
- Other – anyone who identifies a potential need may refer a member to Case Management

- Contact number for person making the referral
- Member/caregiver’s current contact information
- Services you would like coordinated by Case Management
