Eligibility Paperwork

If you are interested in applying for the Lovelace State Coverage Insurance (SCI) please complete the Eligibility Application and take it to your nearest New Mexico Income Support Division (ISD) office.

In addition to the eligibility application, a list of ISD offices in the state is provided below. You will need to take your completed application to one of these offices. The ISD office will make the final determination if you are eligible for the SCI program.

Once the ISD office determines you are eligible for the SCI program, please follow the steps on How To Enroll Tab http://www.lovelacehealthplan.com/SCI/enroll.php. Your SCI benefits do not start until you are enrolled with the Lovelace SCI program and you have been given an effective date.

As an Employer Group

Employers with 50 or fewer employees, who have not discontinued offering health insurance to their employees within the last 12 months, are qualified to participate in the Lovelace State Coverage Insurance program. 

To become enrolled as an employer group in Lovelace’s SCI program, call the Lovelace SCI program at 505-816-6670 in Albuquerque, or 1-888-665-5401 statewide.  Speech and hearing impaired persons may call Relay New Mexico TTY at 1-800-659-8331.
