The more information you provide us when you request a quote, the more accurate our quote will be. Electronic census submission will speed up the underwriting process.

Here’s what we'll need:

Current and proposed plan design
Census on all eligible employees, indicating participating employees (please include dependent status – employee only, family coverage, etc. – and zip codes if employees reside in more than one location in New Mexico.)
Current and proposed contribution strategy
Current and renewal rates by existing carrier
Two years of claim experience if available on accounts with more than 50 eligible employees
