As part of our service to you, we strive to accurately evaluate prospective accounts and provide you with quick turnaround on your requests’ for proposal. When you submit a prospect to be quoted, the more information you can provide us, the more accurate our quote will be. Electronic census submission will also speed up the underwriting process. Whether you are looking for a quote for a virgin group, a full replacement quote or a slice quote, the required information remains pretty much the same:

• Current and proposed plan design
• Census on all eligible employees, with participating employees indicated
  • Zip codes should be included if the employees reside in more than one location in the state
  • Include dependent status (i.e., employee only, family coverage, etc.)
• Current and proposed contribution strategy
• Current and renewal rates by existing carrier
• Two years of claim experience (if available on accounts with over 50 eligible employees)
