These new plans are designed to help you manage your unique health care needs and costs. There are 8 different plan options to choose from, varying upon deductible and co-insurance amounts. All plans allow you the freedom to receive care from both “In-Network” (contracted) providers and “Out-of-Network” (non-contracted) providers and have a Prescription drug benefit built into the plan. Use our Online Provider Directory to view the lists of the contracted Providers and other health care providers in this area who are part of the Lovelace network. Members of the Lovelace iPlan also have the option of adding dental coverage to their plan through our partnership with DentalSource. To help you determine which one of our benefit options best fits your needs and budget, please review our Benefit Comparison Grid (PDF), which lists all the plans we offer. Please also review the premium rate pages, known as the Rate Matrix (PDF), which provides rates based on the benefit option selected, place of residence, age, gender and tobacco use. Once you have had a chance to review and select the option you are most interested in, you are ready to begin the application process. Simply print and complete a Lovelace Individual Plan Enrollment Application (PDF). These forms ask that applicants provide basic information such as name, address, Social Security Number, and details about the applicant’s current medical conditions and medical history.
