Eligibility in Lovelace Senior Plan

• You must be entitled to Medicare Part A and enrolled in Medicare Part B.
• You must live in the Service Area applicable to the product and plan you purchase
• You must continue to pay your Medicare Part B monthly premium
• You cannot have end-stage renal disease unless you are currently a Lovelace Health Plan member through a Commercial group

Lovelace Senior Plan is a Medicare Advantage product available to all Medicare beneficiaries including the disabled.  As a member, you agree to use the services of providers who have gone through our thorough review process and who are part of the Lovelace Senior Plan Network.  There is no health screening on Lovelace Senior Plan.
