Eligibility in Premier Choice

• You must be entitled to Medicare Part A and enrolled in Medicare Part B.
• You must live in the Service Area applicable to the product and plan you purchase
• You must continue to pay your Medicare Part B monthly premium
• You cannot have end-stage renal disease unless you are currently a Lovelace member through a Commercial group

Premier Choice is a Medicare Advantage product available to all Medicare beneficiaries including the disabled. There is no health screening on Premier Choice.
