Interested Provider
Providers interested in participating with Lovelace Health Plan must first complete a Provider Participation Application and submit to Network Operations. Click below for the application and instructions.
Contracting & Credentialing
Once the Interested Provider form is approved, contracting will start both the contracting and credentialing process. You will be notified about the steps to take in the process. Providers are encouraged to utilize the CAQH (Council on Affordable Quality Health Care) on-line application which can be accessed at: https://upd.caqh.org/oas. If the provider does not have a CAQH application number, a number will be provided to you by the credentialing department.
