Interested Provider
Providers interested in participating with Lovelace Health Plan must first complete a Provider Participation Application and submit to Network Operations. Click below for the application and instructions.
Lovelace Health Plan (LHP) is pleased to announce that a new claims payment platform will be in place early this fall. Testing of the new claim platform requires that we delay the effective dates of new provider contracts. This testing and validation of data will assure a smooth transition to this new claims processing system for all existing providers and new providers. LHP will do utmost to keep you updated on the progress of our transitioning to the new claims platform. We apologize in advance for any inconvenience to you or your patients.

Contracting & Credentialing
Once the Interested Provider form is approved, contracting will start both the contracting and credentialing process. You will be notified about the steps to take in the process. Providers are encouraged to utilize the CAQH (Council on Affordable Quality Health Care) on-line application which can be accessed at: https://upd.caqh.org/oas. If the provider does not have a CAQH application number, a number will be provided to you by the credentialing department.
