
Lovelace Health Plan Quality First is an exciting new quality improvement initiative at Lovelace Health Plan. Focused on our Lovelace Medicare Plan members, Lovelace Health Plan Quality First includes several ventures, each centered on improving quality care, reducing costs, and increasing member satisfaction. Lovelace Health Plan Quality First programs are offered at no cost to our eligible Lovelace Senior Plan members.
Care Navigator Team
Lovelace Health Plan has created a special team of care coordinators who navigate care and act as a one-stop information source for our Lovelace Medicare Plan members.
INSPIRIS In-Home Assessments Program
Beginning in April 2011, Lovelace Medicare Plan members who have had little provider contact in the last year have had the opportunity to receive one-time in-home health assessments.
INSPIRIS Care Plus Program
Beginning June 2011, Lovelace Health Plan will offer ongoing in-home care to our most fragile Lovelace Medicare Plan members who have experienced gaps in care.
CareCentrix HomeStar Program: Lovelace Health Plan has partnered with CareCentrix to provide 30-90 days of post-hospital home care services after stays in all LHP contracted hospitals and skilled nursing facilities.
Warm Health Wellness Program: Lovelace Health Plan has partnered with Warm Health, Inc., to provide regular wellness, disease management, and health reminders through an automatic phone system.
Pharmacy Improvements: Pharmacy Services will improve quality by implementing three software systems to improve access to formulary information, to expand the number of members helped by its medication management system, and to streamline the pharmacy authorization process.
